Plan highlights
A POS plan offers you the freedom to choose which doctor you see from our HMO and PPO provider networks. The plan combines the predictable out-of-pocket costs of an HMO plan with the greater flexibility of a PPO plan. When you access care, you can choose to see your primary care physician (PCP), a network PPO doctor, or a non-network doctor.
When you enroll in the POS plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/Independent Practice Association (IPA).

You may need your selected PCP’s ID number when you enroll in the plan for the first time. To find this number, search for your doctor using our Find a doctor tool. Then, click on your doctor's name and select View details under "Primary Care Physician ID."

Behavioral health benefits
The POS plan's behavioral health benefits include inpatient and outpatient mental health and substance abuse care for issues such as:

  • Depression
  • Alcohol/drug abuse
  • Mental illness
  • Marriage and family counseling

If you choose to access care using your Level I (HMO) or Level II (PPO) benefits, then you will only have access to Blue Shield’s mental health service administrator (MHSA) provider network. You will also pay less and receive higher benefit coverage than if you use your Level III (non-network) benefits by going to a non-network provider.

Care outside of California
Through the BlueCard Program, you and your eligible family members have access to care across the United States and around the world.

If you use your Level I (HMO) benefits, you and your covered family members have access to BlueCard providers only for urgent or emergency medical needs or authorized medical follow-up care.

You can use your Level II (PPO) and Level III (non-network) benefits to access care through BlueCard providers for all covered services.

Please note: You are not required to access a BlueCard provider for care. However, using the BlueCard Program can be more cost-effective. It may also eliminate the need for you to pay for the services at the time they are provided, and to submit a claim for reimbursement.

How to estimate your Level II (PPO) medical costs
You can get estimates on the total cost and out-of-pocket expenses for common in-network medical treatments and services. Members can log in and select Treatment Cost Estimator under "Popular Tasks".
